Madison Central Lions Club
Providing Eyeglasses
For a number of years, the Madison Central Lions Club has sponsored providing eyeglasses for those without suitable health insurance and unable to afford the cost. This has long been a program near and dear to MCLC. The current guidelines for obtaining these glasses are listed below.
Eye Exam: We are pleased to report that we now have providers who will do eye exams as part of this program.
Age of Participant: We will serve all ages. School children recommended by school nurses may also participate in the Vision USA Wisconsin Project (see web site). Vision USA uses their own application form, which can be obtained at their web site. MCLC has partnered with the Wisconsin Optometric Association for this project.
Residency: Participants must have established at least twelve (12) months residency in Madison, or have a child in household attending Madison Metro School System.
Previous Lion Assistance: Participants cannot have received previous eyeglass assistance from MCLC in the last two years. We have updated this guideline to better serve Madison residents.
Financial Eligibility: Participants must qualify using the following chart. Referring agencies should request and keep a copy of the applicants previous year's federal tax return.

Application: The MCLC online applications will be used for funding eyeglass grants. Online access to the application is restricted to authorized referring agencies only. For access to the online application, click here.
Authorized Referring Agencies: If your agency would like to be authorized to assist Madison Central Lions Club, one (1) person from your agency should request access to the application web site via e-mail. Only agencies that are willing to comply with MCLC requirements will be authorized, and applications will be monitored to assure compliance.
